The team behind Supported Home.

Supported Home is part of the Civitas Group, an integrated accommodation and care group operating in South East Queensland since 2012. Your care is led by a Registered Nurse and delivered by a small team we employ directly.

Who we are

We are a local team of aged care professionals — nurses, care workers and coordinators — supporting older people across greater Brisbane, Moreton Bay, Logan, the Sunshine Coast and the Gold Coast. Supported Home is a registered provider under the Aged Care Act 2024 (Provider ID PRV-44453) and part of the Civitas Group.

Good care needs the right support behind it. We back our care workers with:

  • practical training
  • rostering and clinical systems
  • experienced clinicians and managers alongside them

Our team is diverse, and that shapes how we work. We take the time to understand a person and what matters to them.

Behind them is a directly employed team of care workers and nurses, supported by our clinical, rostering, quality and people functions. Employing our own people is how the same familiar faces keep turning up, and how we hold one standard of care for everyone who visits.

Clinical governance and quality

Clinical care is led by Wanessa Arraes, our Clinical Lead and a Registered Nurse with 14 years’ community nursing experience. She is accountable for clinical care, care planning and the standard of our nursing.

An independently chaired Quality Care Advisory Body (QCAB) reviews the safety and quality of our care — incidents, feedback and outcomes — and reports to the governing body. It includes a clinical lead and a client representative, so the people who use our services have a real say in how we run.

We work to the Aged Care Quality Standards, report serious incidents under the national scheme (SIRS), and turn what we learn from feedback, complaints and audits into changes to training and how we work.

A Registered Nurse is available through our clinical intake pathway. Referrers can speak directly with our clinical team about discharge planning, complex needs and whether our services suit a person — call (07) 3517 0808 or email nurse@supportedhome.com.au.

Your rights

Set out in the Statement of Rights under the Aged Care Act 2024, the rights of older people receiving aged care are legally enforceable, and we deliver care in line with them.

We give every client a copy of the Statement of Rights when their care begins, and we help them understand what it means for them. It shapes how we plan and deliver support.

If something isn't right, you can raise it with us directly or make a complaint.

Good care fits the person. We support people from culturally and linguistically diverse backgrounds, Aboriginal and Torres Strait Islander people, LGBTI+ elders, and veterans.

We take the time to understand where someone has come from and what shapes their life — language, culture, faith, identity and history — and we build it into their care. Where language matters, we arrange interpreting; where cultural or spiritual practices matter, they belong in the care plan.
