We support referrals across the full range of Support at Home needs, with particular strength in dementia and behaviours of concern, older persons mental health, hospital-to-home transition, and clinical and nursing care at home.
Our directly employed teams are trained in de-escalation, so behaviours of concern are met with a calm, skilled response.

Who we support
We welcome referrals across the full range of Support at Home needs. Our workforce is trained and supported to care for clients with high and changing needs every day. Everyday support referrals are equally welcome. We will work out the right support with you.
We specialise in supporting older people living with:
- Dementia, including behaviours and psychological symptoms of dementia (BPSD) and behaviours of concern.
- Mental illness in later life, including referrals coordinated with existing older persons mental health and AOD services.
- The transition home from hospital, including time-critical discharge support and functional decline after a hospital admission.
- Restorative goals after illness, injury or hospitalisation — including short-term nursing and allied health needs, and restorative care management under the Support at Home Restorative Care Pathway.
- Clinical and nursing needs at home — including medication administration and assistance, and wound care, within our approved capability.
- Everyday support needs — personal care, domestic assistance, social support, transport and getting out — are equally part of our work, delivered as a valued service in their own right.
Working alongside other services
Supported Home provides in-home support and coordination. Where a person also needs mental health treatment, rehabilitation, or alcohol and other drug support, we work alongside the treating teams and services already involved, coordinating care around them.

We deliver in-home support across greater Brisbane, Moreton Bay, Logan, the Sunshine Coast and the Gold Coast. If you are not sure whether we currently reach a particular address, ask us — our coverage is growing.

We work with the person, their family and you, and keep you informed where you stay involved in their care.
Start the conversation — refer now
The best referrals reach us early. You do not need to wait until a patient has been assessed or has funding approved. Talk to us as soon as you see the need: we will work alongside you and the family through the assessment process, line up the right support, and be ready to begin the moment funding is in place.
If a patient has not yet started the aged care assessment process, it begins with My Aged Care: myagedcare.gov.au/apply-online or 1800 200 422.
Our response commitment
We respond to new referrals by the next business day. The fastest routes are the online referral form and the clinical intake line, staffed during office hours (9am–5pm, Mon–Fri).
Support workers on evening and weekend shifts are backed by our on-call arrangements — an internal arrangement, not a referral or intake channel.

Deal direct with our clinical team. Our Clinical Lead, Wanessa Arraes, is a Registered Nurse with 14 years’ experience in community nursing across aged care and complex disability. We take referrals across the Support at Home pathways, including:
- The Restorative Care Pathway, including restorative care management — for restorative goals after illness, injury or a hospital admission.
- The Assistive Technology and Home Modifications scheme — for equipment, products and home adjustments.
- Ongoing Support at Home services — everyday and personal care, nursing and clinical care, allied health, and social support.
If you are referring through a particular pathway, tell us — we will set the referral up accordingly.
