Some homecomings need a few weeks of help after surgery. Others need a fuller restorative plan. We build support around the first stretch home.

The first days home

The first weeks at home shape the whole recovery. We are there from day one, with personal care, medication prompts, meals and help moving safely around the house. It is the steady support that keeps small problems from becoming a trip back to hospital.

Help at home after surgery or a procedure

After a hip, a knee, a cardiac procedure or a longer admission, you may need help at home after surgery for a few weeks. We cover showering and dressing, getting to follow-up appointments, and keeping on top of new medications. We also watch for the things that send people back to hospital. We set it up quickly, then ease it back as you recover.

Getting back on your feet (restorative care)

Some homecomings are about getting stronger again. Short-term restorative support is goal-oriented. It is a focused plan to rebuild strength and confidence after illness or injury, so you get back to doing more for yourself. Supported Home is registered to deliver this support under both the Transition Care Programme and the Support at Home Restorative Care Pathway, usually following an ACAT assessment. These pathways are time-limited. We help you plan ahead for what comes next, and if ongoing help is useful, we can continue it through our reliable home care.

A plan built around getting home safely

We confirm what is needed, from personal care and nursing to medication, mobility and transport to follow-ups. Then we put the right people in place, with Registered Nurse oversight where there is a clinical need. It is the same small, directly employed team, so a familiar face turns up each visit.

Support after hospital is designed to taper. We review as you improve and adjust as you need less. We also help you and your family plan for what happens when short-term funding ends. We coordinate with your hospital team, GP and ACAT throughout.

What should we expect in the first days and weeks after discharge?

What is the Transition Care Programme or the Restorative Care Pathway?
These are recognised, time-limited pathways that help an older person regain strength and independence after a hospital stay, usually following an ACAT assessment. Supported Home is registered to deliver support under both. Eligibility and funding are decided through the assessment process. Our clinical team can discuss whether our services may suit the person’s needs.
Who can make a referral?

Do the same carers come each time?
As much as possible, yes. Our care workers are directly employed by Supported Home, so we can send a small, consistent team who get to know the person. That continuity matters most while someone is recovering.
