A Supported Home care worker providing home care to an older person at home

Our care workers are directly employed by Supported Home, so a small, consistent team comes to know the person and how they like things done. When a regular care worker is away, cover comes from the same familiar team.

We cover greater Brisbane, Moreton Bay, Logan, the Sunshine Coast and the Gold Coast.

Home care services for everyday life

The ordinary, essential help that keeps someone comfortable and independent at home.

Personal care

Help with the personal things, done with dignity and at the person’s pace:

  • showering, dressing and grooming
  • getting ready for the day

The same care workers each time, so it stays comfortable and familiar.

Help around the home

Keeping a home running:

  • cleaning, laundry and changing the bed
  • a hand with the jobs that have got harder

Domestic assistance that keeps the place comfortable, tidy and safe.

Getting out and about

We provide transport and go along where it helps:

  • medical appointments and the shops
  • social outings and regular activities

Leaving the house stays easy and safe.

Meals and shopping

Meals prepared the way the person likes them:

  • a hand with the weekly shop
  • planning, cooking and keeping the kitchen stocked

Eating well stays simple.

Companionship

Company matters. A familiar face for:

  • a cup of tea and a chat
  • a walk or a shared hobby
  • regular contact through the week

Companion care keeps people connected, which is good for mood and health.

Respite for family carers

If you care for someone yourself, you need a break sometimes.

  • in-home respite for a few hours or a few days
  • with people who already know the person

Their routine stays steady while you rest, work or get away.

A team you know, support planned around the person

The same small team

We build a small team around each person and keep it consistent, so the care workers know the person, their home and how they like things done. Continuity is the whole point of reliable home care.

Support planned around the person

We plan the support together and adjust it as things change, from a couple of hours a week to daily visits.

Dependable scheduling

We plan cover for leave so visits go ahead as arranged, and if a visit has to change we tell you early. That is what reliable means to us.

Frequently asked questions

What’s included in home care?
Home care covers the everyday help that keeps someone comfortable and independent at home: personal care, help around the home, transport to appointments and outings, meals and shopping, companionship, and respite for family carers. We plan it around the person and adjust it as things change. Call (07) 3517 0808 to talk through what would help.
Will the same care worker come each time?
As much as possible, yes. Our care workers are directly employed by Supported Home, so we build a small, consistent team who get to know the person. If a regular care worker is away, someone from that same team steps in, a face the person already knows.
What areas do you cover?
We provide home care services across greater Brisbane, Moreton Bay, Logan, the Sunshine Coast and the Gold Coast, and the wider South-East Queensland region. If you are not sure we reach a particular suburb, call (07) 3517 0808 and we will let you know.
How does funding work for home care?
For eligible older people, in-home care can be funded under Support at Home, the Australian Government’s program for aged care at home. Funding follows an assessment, and eligibility is decided through My Aged Care, not by us. We can help you understand what the funding can be used for and what the next step is. Call (07) 3517 0808.
Do I need an assessment or a referral to start?
No. You can talk to us before any assessment or referral, and we can arrange support privately, without waiting for funding. If you would like funded support, an assessment is part of that process and we can point you to it. Call (07) 3517 0808.
Can you help with respite so I can have a break?
Yes. We provide in-home respite for a few hours or a few days, with people who already know the person, so their routine stays steady while you rest, work or get away. For dementia-specific respite, our dementia service can help too.
Can you change the support as needs change?
Yes. We plan the support together and review it as things change, from a couple of hours a week to daily visits. If needs become clinical, our registered nurses can step in, and we can connect you to our other services.
What happens if our regular care worker is away?
We plan cover for leave and illness, so visits go ahead as arranged. When a regular care worker is away, someone from the team who already knows the person steps in, and we let you know in advance where we can.
