Clinical care, brought into the home

From a single visit for a dressing change to ongoing clinical support, the care comes to you.

Wound care

Wounds heal better with the right care done consistently. Our registered nurses manage dressings, surgical wounds, pressure injuries and chronic wounds at home, on a schedule that suits the person, and they watch for the early signs that a wound needs review. We coordinate with the GP or hospital team treating it.

Medication administration

Medications get complicated, especially after a hospital stay or with several conditions to manage. Our registered nurses administer and review medications, manage webster packs and injections, and check that what was prescribed is what is actually being taken. We coordinate with the GP and pharmacy.

Catheter and continence care

Continence changes and catheters are common in later life, and they are very manageable with the right clinical care. Our registered nurses provide catheter care and changes, and support continence with dignity and discretion, at home. We watch for the early signs of infection or other problems that need review, and we coordinate with the GP.

Ongoing health support

Living with a chronic condition, or several, means keeping an eye on the things that matter: blood pressure, blood sugar, skin, continence, weight and mood. Our registered nurses monitor health at home, catch changes early, and adjust the plan with the GP before a small problem becomes a hospital admission.

After surgery or a hospital stay

Recovery often needs clinical care for a while after a procedure or an admission. We provide post-operative nursing at home: wound and drain care, pain relief and medication administration, and monitoring for complications, working with the hospital team. When the whole homecoming needs support, our coming home from hospital service brings it together.

Registered nurses, a team you know, working with your GP

Delivered by registered nurses

Every nursing visit is delivered by a registered nurse, and the care plan is written and reviewed clinically. When something changes, a nurse is the one who sees it and acts.

The same skilled team

Our nurses and care workers are directly employed by Supported Home, so the person sees familiar faces who know their history and build on it each visit. They are backed by an on-call arrangement after hours, so cover and continuity hold. Continuity makes clinical care safer.

Alongside your GP and treating team

We coordinate with the GP, hospital team, pharmacy and any specialists involved. Our nursing care adds to the medical care already in place, and everyone works from the same plan.

What nursing care at home costs (and how funding can help)

Cost is usually the first question, and it is a fair one. The honest answer is that it depends on how much nursing is needed and how often.

For eligible older people, in-home nursing is funded as clinical care under Support at Home. Clinical supports including nursing carry no co-contribution, which means eligible participants are not asked to contribute toward the cost of their funded nursing visits, following an assessment of what the person needs.

Where someone is not eligible, or would rather arrange care privately, we also provide nursing on a private basis, with clear, itemised costs given before anything starts.

Do I need a referral or an assessment to get a nurse at home?
No. You can talk to us before any referral or assessment, and we can arrange private nursing straight away. If you want care funded under Support at Home, an assessment is part of that process, and we can point you to it. Call (07) 3517 0808.
