You don’t need a referral, or your funding sorted, to talk to us.

Our nursing care is delivered by registered nurses and led by Wanessa Arraes, our Registered Nurse and Clinical Lead, working alongside the GP and treating team already involved.
We cover greater Brisbane, Moreton Bay, Logan, the Sunshine Coast and the Gold Coast.
Clinical care, brought into the home
From a single visit for a dressing change to ongoing clinical support, the care comes to you.
Wound care
Wounds heal better with the right care done consistently. Our registered nurses:
- manage dressings, surgical wounds, pressure injuries and chronic wounds at home
- watch for the early signs a wound needs review
- coordinate with the GP or hospital team treating it
Medication administration
Medications get complicated after a hospital stay, or with several conditions at once. Our registered nurses:
- administer and review medications, webster packs and injections
- check that what was prescribed is what is actually being taken
- coordinate with the GP and pharmacy
Catheter and continence care
Common in later life, and well managed with the right clinical care. Our registered nurses:
- provide catheter care and changes at home
- support continence with dignity and discretion
- watch for early signs of infection and coordinate with the GP
Ongoing health support
Chronic conditions need a steady eye on the things that matter. Our registered nurses:
- monitor blood pressure, blood sugar, skin, weight and mood at home
- catch changes early
- adjust the plan with the GP before a small problem becomes a hospital admission
After surgery or a hospital stay
Recovery often needs clinical care for a while. Our post-operative nursing at home covers:
- wound and drain care
- pain relief and medication administration
- monitoring for complications, working with the hospital team
When the whole homecoming needs support, our coming home from hospital service brings it together.
Registered nurses, a team you know, working with your GP
Delivered by registered nurses
Nursing visits are delivered by registered nurses. Wanessa Arraes, our Clinical Lead, writes and reviews every care plan. When something changes, a nurse sees it and acts.
The same skilled team
Our nurses and care workers are directly employed by Supported Home, so the person sees familiar faces who know their history. When a nurse is away, another nurse from our team covers the visit. Continuity makes clinical care safer.
Alongside your GP and treating team
We coordinate with the GP, hospital team, pharmacy and any specialists involved, and everyone works from the same plan. When a visit raises a concern, our nurses contact the GP or treating team directly.
What nursing care at home costs (and how funding can help)
Cost is usually the first question, and it is a fair one. It depends on how much nursing is needed and how often.
For eligible older people, in-home nursing is funded as clinical care under Support at Home, with no contribution toward funded nursing visits. Funding follows an assessment of what the person needs.
Not eligible, or would rather arrange care privately? We provide private nursing, with clear, itemised costs before anything starts.
