We collect personal information that helps us deliver Support at Home aged care services. This may include your name, contact details, date of birth, health information, emergency contacts, and other details needed to support your care.

How we collect it

We usually collect information directly from you or your authorised representative. Sometimes we receive information from other providers, carers, or health professionals involved in your care.

How we use your information

We use your information to:

  • Provide you with safe and effective services and supports.
  • Meet our legal and funding obligations under the aged care system.
  • Improve our services and manage our relationship with you.

Your sensitive information, such as health or cultural information, is only collected and used with your consent or as required by law.

Who we share it with

We may share your information with:

  • Government agencies such as My Aged Care and the Aged Care Quality and Safety Commission.
  • Health care providers and other parties involved in your care.
  • Trusted contractors and service partners who help us deliver services.

Your information will not be shared overseas without your consent, unless allowed by law.

How we keep it safe

We store your information securely, in paper and electronic formats, and take all reasonable steps to protect it from misuse, loss or unauthorised access.

How we use AI

We use general artificial intelligence tools to help our team with everyday tasks, such as drafting and summarising information, so we can spend more time on your care. AI does not make decisions about your care. A member of our team always reviews the work and makes the decision. We do not put your personal information into public AI tools without appropriate protection, and our staff follow the Privacy Act 1988 (Cth) and our privacy obligations when they use these tools.
